Telomerase reverse transcriptase (abbreviated to TERT, or hTERT in humans) is a catalytic subunit of the enzyme telomerase, which, together with the telomerase RNA component (TERC), comprises the most important unit of the telomerase complex.

The TERT gene provides instructions for making one component of an enzyme called telomerase. Telomerase maintains structures called telomeres, which are composed of repeated segments of DNA found at the ends of chromosomes.

In this review, we summarise the function and structure of telomerase reverse transcriptase (TERT) in humans, including its regulation. The dysregulation of telomerase through TERT promoter mutations across a range of cancers is discussed. The molecular mechanism activated by TERT promoter mutations is outlined.

Data indicate that the telomerase reverse transcriptase (TERT)(TERT)-DNA methyltransferase 3B (DNMT3B)-PTEN-AKT axis provides an explanation for multi-oncogenic activities of TERT and may be exploited in hepatocellular carcinoma (HCC) treatment.
