The kidneys reveal echogenic renal medullary pyramids. No convincing acoustic shadowing is noted to suggest calculi. There are three rounded cysts in the medullary region of the right kidney- one in the upper pole and two in the lower pole- with internal debris.

Medullary sponge kidney occurs because of focal or diffuse dilation of the medullary collecting tubules with stasis of urine and may eventually lead to calculi deposits. On ultrasound exam it appears as hyperechoic calyces when stones begin to form in the dilated collecting tubules.
